Behavior
The volume is the mechanism. Everything it does is noise at a register that makes coherent thought impossible. The screaming is not pain — it sounds like pain, which is different. Whatever created it understood acoustics and made a considered choice.
Appearance
Has the proportions of a person who gave up on being a person some time ago and is now mostly voice. You hear it several floors before you encounter it, which is either a warning or not, depending on the layout.
Physical description
Human-shaped corpse, slumped and hollow-chested, skin desiccated and grey-brown like old leather. Ribs visibly flexed outward, sternum cracked open into a resonating cavity. Jaw unhinged far beyond normal range, throat distended and pulsing. Stands slack and still between screams, head tilting toward sound sources.
